NEOS participates in most insurance plans. Please bring your insurance card and any required referrals with you to each visit. Please be prepared to take care of your co-payment at the time of your visit. If you have a work injury, please call the office with the date of injury, your employer’s name and workers’ compensation insurance information prior to booking the appointment so that we may obtain approval for treatment and for the payment of our fees. If you have a motor vehicle injury, please bring all relevant information to your first visit.
