Patient Portal
The NEOS Patient Portal is intended to be a tool to increase convenience for our patients. Once you register, you will be able to request appointments, refill medications, make payments on your account and ask questions. You will also be able to access a summary of the care you have received here. For a full copy of your medical records, you will need to sign a release form and mail or fax it to our Medical Records Department.
How to Register for the Patient Portal:
For privacy and security reasons, the NEOS Patient Portal is restricted to established patients only. If you are a new patient, you will be eligible to become a Patient Portal user as soon as you have registered in person at our practice. Just ask any of our staff for your Patient Portal Registration Letter. This is a letter that has a unique identifier for you and has instructions for the online registration process. If you are an established patient just call 413-785-4666 and tell them you would like to register for our Patient Portal.
